Payment Terms & Policies for BODYWORK
Cash and credit cards accepted. All payments are due at the time of service no exceptions. IF, fees are charged to the account must be brought back into good standing before another service will be conducted.
**Effective 04/01/2023 Square credit card processing is no longer refunding transaction fees to the customer or businesses for any refund amounts.
Insurance: No private insurance coverage is accepted including any government payments such as Medicare. You can request a receipt and treatment notes that can be submitted for reimbursement. HSA cards are welcome but if they do not process have another form of payment ready.
No Call / No Show Policy: 5o% charge to card used during booking.
Cancel within 2 hours: 50% charged to card used during booking.
If you are LATE you have two options: 1. Come in for your remaining time and pay the full amount. 2. You may ask if the schedule allows you to extend your session to get your full time at no extra charge.
Payment Terms & Policies for Classes
General Policies for classes for individual & group classes:
Any classes can be booked or canceled at 72 hours without penalty. Blended classes each student must complete the online portion in order to be accepted into the in person skills practice (proof must be provided to the instructor at the time of arrival to the class.
It is the responsibility of the learner to ensure they have paid for the correct class. If you have questions before you purchase, please reach out via email and ask your question to ensure you are purchasing the correct item for your needs.
Auditing of a course is not permitted.
Childcare is not provided within the learning space.
It is recommended that all students arrive 15 minutes early to verify they are in the correct location. All classes start when scheduled, registration and check-in is done before the start time. Those individuals who arrive more than 5 minutes after the start time of the class will not be allowed to enter and complete the course (this is required by the citification providers not this training provider). Being late, no call no show, for class will result in no certification along with no refund of any deposits.
Individuals 14 years of age or older must be signed up by a parent or legal guardian, and the method of payment must be conducted by the parent or guardian. On the day and time of the course, parent or guardian should ensure that the instructor is aware that they have arrived and that the student has been signed in for the class and that they know when to return back at the end of the class.
If a class fails to meet the minimum requirement of 4 students it will be rescheduled, and students can reschedule online if needed students can get a refund. Students will be notified as soon as it becomes apparent the four student threshold cannot be met.
Refunds exclude credit card processing fees if the card is charged. This provider will not be responsible for extenuating cost related to transportation cost lodging parking fees, if a class cannot be held due to extend circumstances or if the student is late and cannot be certified.
In the case of the site becoming unavailable due to weather or extenuating circumstances, if the instructor is unable to conduct the class, individuals will be notified in a reschedule date will be announced.
Online Only Individual Non-Certification:
Individuals pay and sign up online. You will then receive a class link tied to your email submitted up upon purchase and you take that class online. Upon completion of the online course, Once a class link is sent to the email address and clicked on it is not refundable. It is highly recommended that that class link be clicked on no later than 5 days after purchase once it is received through email to avoid any complications with the link validation expiring.
Individual Joining a Independent Class:
A credit card number is entered online to reserves your space, and is your final form of payment. Once a Blended class clink is sent and it is clicked on it no longer refundable.
You can book or cancel your part in the class up to 72 hours before the classes start time without charge online using the confirmation e-mail. If you do not call or show up to the class your card will be charged your class fee in full.
Group Classes for Organizations:
You can book or cancel your part in the class up to 72 hours before the classes start time without charge online using the confirmation e-mail. A credit card number is entered online to reserves your space. An invoice is your final form of payment. Valid Credit Card holds the date and time and will be changed ONE STUDENT FEE for a late cancel.
The final cost is of the class is calculated as follows: The number of all students listed in the booking form - the one student deposit + optional Knifes add on section = final payment. The invoice is due within 14 days of the class completion date. Any invoice is not paid within 21 days from the class date maybe be sent to collections and the organization will be blacklisted from being able to schedule any courses to this provider in the future.
Individual students cannot be rescheduled when classes are organized by group or organization. Students can schedule themselves to another class. If there is a case of multiple classes happening in one day for the same group and there is an opportunity for the student to go to the other session, they can do so however a classroom ratio cannot exceed 15 students to 1 instructor for AVIRT classes. The ratio is 12 to 1 for CPR. This is a requirement of the citification organization.
If it is a blended delivery, once a student has been sent their link to the blended online portion and they click on that button their portion cannot be refunded. If they do the blended portion, but do not come to the skills practice that money will not be refunded. This is due to the fact that the training center must purchase the individual student links to a blended class in advance from the parent company HSI. Their policy states that once a link is clicked on, it is not refundable the training center must abide by this as well.
Mobility Accommodations: If you have a mobility concern getting into the treatment room please contact Tony before booking (email is preferred). If it is determined that it would be impossible to get you into the treatment room an accommodation could be made to come to you (at no extra charge). There are 18 steps and one turn at the top of the steps. The following conditions would need to be met for an accommodation: Is there space to set up a table? Is the location within reasonable proximity to the office? A session time could be accommodated with enough notice in most cases.
Those Under 18: Tony Schwartz Bodywork will refuse service to anyone under the age of 18 without an adult consent. Anyone between 16 and 17 will need a signed parent or guardian consent form & intake form. Anyone under 15 must have a parent or guardian in the room or in the waiting room at all times during a service.
Intakes: You must fill out an intake form upon your first visit. No service for anyone who does not fill out all required information on the intake form.
Draping: By city ordinance you are required to be draped at all times. You may always request to have the door to the office (left open while you are fully clothed) for your session.
Your Personal Information: Your intake forms, session notes, and direct electronic correspondence are protected by HIPAA. I make a point to keep non-electronic files in the age of hackers and identity theft that follows HIPAA standards. I prefer passive marketing such as social media which allows you to set the terms of when you want to have contact with me. Many of my clients appreciate the fact that they don't hear from me until they contact me first.
Right to Client Refusal: Tony Schwartz Bodywork may refuse service to anyone or limit service to anyone who has health conditions that could cause harm to either party. Tony may refuse service before a session for any reason that could compromise the safety of the practitioner.
FAQ's
Q: I have never had a session before and I am unsure of the edict. What are some things I should know?
A: Clothing: You should only undress to your comfort level (anywhere from nude to fully clothed). You will never be nude without a sheet.
A: Personal Hygiene: Please take a shower before coming. If you have any contagious skin conditions please inform me upon your arrival so that I may protect myself. Also, please contact me if your home or office is being treated for lice, bed bugs, or other such things as your session will need to be rescheduled.
Q: You have a private office and I am a female is it safe to come?
A: If you would feel better about coming to see a male therapist if you brought someone else with you to sit in the waiting room or join you in the room during a service that is fine with me. Another suggestion would be to leave a note with someone that tells them where you are and your start and end time. When your session is over contact them to let them know you are okay (I used to use a similar system of addresses and a call back time for my own safety on out-call sessions). Lastly, you can always request that the door to the office be left open. Almost all services I provide can be done with clothing on. Massage is health care and as such requires a professional to act with integrity and professionalism that also provides a sense of safety and respect for a client. I promise to make every effort to make that possible.
Q:How far in advance should I request a session?
A: I tell people about one week is perfect…sooner than that just means you might not get the time you want. DO NOT call for last minute sessions. If you do not see it on the website booking system its not available.
Q: I am a guy, what happens if I have an erection?
A: Yep… It happens and I don’t care. Men have erections at random all the time. It does not send me any “signals” nor need give you any reason to be embarrassed or concerned.
Q: If I don’t like the session what happens?
A: Massage and bodywork is a very personal thing and finding the right therapist for you can take time and trying a number of therapists. I never get offended by someone communicating the thoughts about their sessions with me. First and foremost, please say something during the session. I am not a mind reader but people often expect it. If after a session you don’t feel like you got the session you were hoping for, please contact me. Your honesty could lead to much better next session or I can point you in the direction of someone who might be able to serve you better.
If you ever feel a therapist has violated you in someway, that is a criminal act, never hesitate to contact the police immediately and have a report made so that investigation can be completed. In most locations around Minnesota if therapist is required to be licensed and a complaint is made, that office is typically automatically notified. I fully support any client who feels that they have been mistreated to contact the authorities immediately.
Q: I saw something online that I thought might help me. Will you or can you do that?
A: Many treatments are condition dependent so I would need to make sure the treatment fits your condition and that I know how to perform such a treatment. Also ask because it could save you a lot of time and perhaps save you from making pain worse.
Q: I hate being marketed to and I hate intake forms so can I skip it?
A: The short answer is no. The state of Minnesota and my insurance carrier requires documentation of your sessions in the event of a claim. In the case of product recalls, and health department requirements I must have all of the information requested in the intake form. My marketing policy is simple: I do not contact you until I have to. I use passive marketing methods only like social media. I encourage everyone to look at one media source occasionally to keep up to date.
